![]() The Community Healthy Weight Network Action Guide is the result of a three-year collaboration between the University of South Carolina School of Medicine and the Duke University Medical School. The objective of the Action Guide is to provide community-based groups with the tools needed to promote healthy weight in their communities. The Action Guide provides tips on how to build a community network that can promote better nutrition, increased physical activity, and healthy weight. In addition, examples of community-based programs that resulted from these network activities are given for communities in both South and North Carolina. The USC School of Medicines' Rural Primary Care Education Projects provided the leadership for healthy weight community-based programs in the counties of Fairfield, Lancaster, and Marlboro.
